Barriers to care coordination: separating myth from reality
Abstract
Cut through the barriers to care coordination. There are plenty of obstacles getting in the way of effective care coordination, but physicians report that one of their biggest concerns is the fear that taking on care coordination responsibilities will somehow increase their exposure to liability. However, the evidence suggests this conventional wisdom is, in fact, far from reality.
