Conflict of interest statement
Competing interests: The author is a doctor and experienced a life threatening serious adverse event after elective laparoscopic surgery in an NHS hospital. This is the subject of a complaint and (following failure of resolution) an ongoing legal action. He is also concerned that his experiences may be avoidably replicated in hospitals throughout the UK up to 50 times a year, sometimes fatally.
