The design of the multilayered electronic health record for the Telecare Pilot Project in Taiwan

Abstract

The three-layered electronic health record (EHR) template is built to assist the Telecare services information sharing across the long term care spectrum. Based on the urgency of the information needs, providers can access the most important and updated health information at the first layer of the EHR. The single health encounter summary and the history of the outcome of a specific health assessment have been presented at the second and the third layer.
