Abstract

The ethical context of prematurity is a special one. Today, patient care involves many players. In addition, parents are better informed and demand that their opinions be considered in the therapeutic options available to their prematurely born infant. Such a situation brings into play a number of ethical principles, including the central notion of the child's best interest. In the case of a premature infant whose viability is borderline, more importance should be placed on discovering how to achieve consensus among the parties concerned than deciding who should make the decisions. The present article proposes a description of the steps that might lead to a consensus decision.

Le contexte éthique de la prématurité est particulier. Aujourd’hui, plusieurs intervenants s’engagent dans les soins des patients. De plus, les parents sont de mieux en mieux informés et demandent que l’on tienne compte de leurs opinions dans le choix thérapeutique offert à leur enfant prématuré naissant. Dans une telle situation, plusieurs principes éthiques entrent en jeu, dont la notion centrale de l’intérêt primordial de l’enfant. Il devrait être plus important de découvrir comment arriver à prendre une décision consensuelle entre les différents intervenants que de décider qui doit prendre la décision dans le contexte de la naissance à la limite de la viabilité. Cet article propose une description des étapes pouvant permettre une décision consensuelle.
