Abstract

We report the effects of unilateral recession-resection surgery of the horizontal recti muscles with inferior displacement and augmented anterior transposition of the inferior oblique muscle with a posterior intermuscular suture in a patient with large exotropia and considerable hypertropia.

Figures

Fig. 1
Preoperative and postoperative alignments in a patient with hypertropia and large exotropia. Preoperative exotropia of 80 prism diopters (PD) and hypertropia of 20 PD in the right eye (left) were improved to 10 PD of right exotropia in the primary position one week postoperatively (middle). At five-months postoperative, a 30 PD right exotropia had redeveloped (right); however, the exotropia was stable.

Unilateral recession-resection surgery with inferior displacement and augmented anterior transposition of the inferior oblique (IO) muscle with a posterior intermuscular suture. Supramaximal lateral rectus muscle recession of 12.0 mm with 4.0 mm of inferior displacement (left). Augmented anterior transposition of the IO muscle with a posterior intermuscular suture 6.0 mm posterior to the muscle insertion (right).
