[Two-stage surgical method of treatment of locally disseminated cancer of the thyroid]
Abstract
The article presents immediate and long-term results of treatment of 74 (15.9%) patients with disseminated regional metastasing of the thyroid cancer from a group of "high operative risk". It was shown that in elderly patients having many coexisting diseases with vast dissemination of the primary tumor with invasion to the surrounding organs and tissues and involvement of lymph nodes on the neck on both sides and in the mediastinum the operations should be divided into 2 stages. It supposes thyroidectomy along with the central and lateral lymphadenectomy as the first step. Lymphadenectomy on the other side of the neck and/or transsternal lymphadenectomy should be performed within 2-3 months after correcting therapy. Such approach did not worsen the long-term results and decreased postoperative lethality and number of postoperative complications.
