Prediction of serum ionized calcium concentration by serum total calcium measurement in cats
Abstract
Feline serum samples (n = 434) were classified as hypercalcemic, normocalcemic, or hypocalcemic based on both total calcium (tCa) and ionized calcium (iCa) concentrations. Sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), positive diagnostic likelihood ratio (PDLR), and negative diagnostic likelihood ratio (NDLR) were calculated for prediction of hypercalcemia and hypocalcemia in all samples, in hypoalbuminemic cats, and in those with chronic renal failure (CRF) as compared with cats that had other conditions. Diagnostic discordance in prediction of iCa using tCa was 40%. Sensitivity of tCa in prediction of ionized hypercalcemia was low and specificity was high. The PDLR for prediction of ionized hypercalcemia or hypocalcemia was low in all cats, especially in those with CRF. Due to the high level of diagnostic discordance, tCa should not be used to predict iCa concentration. Concentration of iCa should be measured directly when accurate assessment of calcium status is needed.
Des échantillons de sérum félin (n = 434) ont été classés comme étant hypercalcémique, normocalcémique ou hypocalcémique en se basant sur les concentrations du calcium total (tCa) et du calcium ionisé (iCa). La sensibilité, la spécificité, la valeur prédictive positive (PPV), la valeur prédictive négative (PNV), le ratio de possibilité de diagnostic positif (PDLR) et le ratio de possibilité de diagnostic négatif (NDLR) ont été calculés pour la prédiction de l’hypercalcémie et de l’hypocalcémie dans tous les échantillons, chez les chats hypo-albuminémiques, et chez ceux avec une défaillance rénale chronique (CRF) comparativement à des chats qui avaient d’autres conditions. Une discordance diagnostique dans la prédiction d’iCa en utilisant tCa était de 40 %. La sensibilité de tCa pour la prédiction d’hypercalcémie ionisée était faible et la spécificité était élevée. Le PDLR pour la prédiction d’hypercalcémie ionisée ou d’hypocalcémie était faible chez tous les chats, spécialement chez ceux avec CRF. Étant donné le haut niveau de discordance diagnostique, tCa ne devrait pas être utilisé pour prédire la concentration d’iCa.
