Electrocardiogram after electrical cardioversion shows sinus rhythm with shortened P-R interval (0.1 s) and widened QRS complexes (0.14 s) with typical delta waves (paper speed, 50 mm/s). Arrows indicate delta waves (positive in leads I, aVL, and V2–V6; negative in leads III, aVF, and V1). Negative delta wave and QRS complexes in lead V1 (along with positive delta wave and QRS complexes in leads I, aVL, and V2–V6) point to a right-sided accessory pathway. Negative delta wave and QRS complexes in leads III and aVF point to a posteroseptal location of the accessory pathway in the right ventricle.
