Figure. Role of the clotting cascade in atherosclerosis
The clotting cascade can be divided into initiation and amplification phases that are analogous to a stick of dynamite with a fuse and an explosive. The tissue factor/factor VIIa (FVIIa) complex initiates coagulation and generates small amounts of thrombin before being inhibited by tissue factor pathway inhibitor. Thrombin then activates different components of the cascade that allows amplification of the clotting cascade and the generation of large amounts of thrombin via the intrinsic pathway and the prothrombinase complex (FVa/FXa). FXI is also activated by FXIIa. Thrombin may enhance atherosclerosis via multiple pathways.
