Abstract

AT UNIVERSITY HOSPITAL Birmingham NHS Foundation Trust, which is also taking part in the second phase of the PAC programme, we chose to tackle the provision of discharge information for older patients because they make up the majority of the 500,000 patients we see at our two sites every year.
