Multiple gastrointestinal factors could influence the insulin secretory response to a meal challenge. These include the rate of gastric emptying and other factors that directly or indirectly affect motility, enteroendocrine secretion, and intraluminal digestion. Tests of β-cell function may challenge different components of the insulin secretory apparatus. However, it is important to remember that the β-cell secretory response (Φ) must be considered in light of the insulin action (Si), as illustrated in the figure, where identical Φ reflects two different states of glucose tolerance. IGT, impaired glucose tolerance; NGT, normal glucose tolerance.
