Normal cardiac lymphatic flow is from endocardium to subepicardial collecting lymphatics. The chronic response of lymphatic vessels after transplantation is marked by proliferation and dilation of collecting lymphatics permitting greater egress of antigen presenting cells (APCs) from the donor heart to recipient draining lymph nodes. The subsequent immune response results in cardiac allograft vasculopathy (CAV). During the acute response to myocardial ischemia there is a robust lymphangiogenesis that facilitates repair of infarcted myocardium.
