Ivabradine is FDA-approved for patients with stable heart failure and an ejection fraction of 35% or less to reduce the risk of hospitalization due to worsening heart failure. By selectively inhibiting the I(f) current in the sinoatrial node, ivabradine lowers heart rate without affecting myocardial contraction, relaxation, or ventricular repolarization. Reducing resting heart rate is a therapeutic strategy to mitigate adverse cardiovascular outcomes, particularly in patients with left ventricular dysfunction. This activity reviews ivabradine’s indications, administration, contraindications, and precautions to optimize clinical decision-making.
