(A) Trocar placement. 3 arms are placed in the same alignment, around the left anterior axillary line, the right arm (arm 1), the camera holder (arm C) and the left arm (arm 2) are respectively inserted through the 2nd, 4th, and 6th intercostal space. The 4th arm (arm 3) is placed at half distance of a line drawn from the xyphoid appendix and the left trocar line. (B) Distal anastomosis. It starts from outside the internal mammary artery to inside the coronary artery and the running suture continues anticlockwise (blue arrows), intracoronary shunt in place.
