Pancreas transplantation followed by stumpless duodenal exocrine drainage was performed in 2 patients. Primary kidney and pancreas graft function was noted in both cases. Two procedures of pancreas transplantation followed by stumpless duodenal exocrine drainage were carried out for the first time. Perhaps, absent donor duodenal stump has several advantages for the further course of postoperative period that may be confirmed in prospective trials.

Двум пациенткам выполнена сочетанная забрюшинная трансплантация почки и поджелудочной железы с безкультевым дуоденальным дренированием экзокринного секрета трансплантата. В обоих случаях отмечена первичная функция трансплантата почки и поджелудочной железы. У одной пациентки послеоперационный период протекал гладко, у другой ранний послеоперационный период осложнился развитием мелкоочагового ишемического панкреонекроза трансплантата с образованием парапанкреатического жидкостного скопления, успешно купированного миниинвазивными санационными методиками. Возможно, отсутствие культи донорской двенадцатиперстной кишки как таковой обладает рядом преимуществ для дальнейшего течения посттрансплантационного периода, что может быть подтверждено в проспективной серии наблюдений.
