[The mechanism of arousal in patients with obstructive sleep apnea-hypopnea syndrome]
Abstract
阻塞性睡眠呼吸暂停低通气是由于睡眠期间反复发生的上气道部分或完全阻塞,导致频繁的气流受限并伴随血气变化(主要包括氧气和二氧化碳的变化),其病理生理学机制包括解剖及非解剖因素,其中觉醒阈值改变作为非解剖因素参与其中。大约有30%~50%的阻塞性睡眠呼吸暂停患者觉醒阈值降低,低觉醒阈值增加夜间觉醒次数,保护患者避免夜间血氧过低,但对部分患者而言,反复觉醒导致睡眠片段化,从而破坏睡眠结构,降低呼吸稳定性而加重病情。深入探索阻塞性睡眠呼吸暂停低通气综合征的觉醒机制可以为临床制定个体化治疗提供新思路,从而获得最佳的治疗效果。.
