Schematic of laminar and turbulent flow regimes exert differential forces on VWF multimers. The force exerted by the two regimes yield differences in the exposure of the cryptic ADAMTS13 cleavage site in the A2 domain (open circles), resulting in two distinct multimer distribution profiles (adapted from ref. 16). Turbulent flow regime results in excessive loss in ultra large VWF multimers (ULVWF), reminiscent of multimer distribution in type 2A VWD.
