Schematic depiction of levels of enthusiasm developing for surgical revascularization and/or endovascular stenting for the treatment of renovascular hypertension. Early enthusiasm for PTRA with/without stenting became tempered by inappropriate patient selection, occasional complications, more effective medical therapy and the results of limited prospective trials. The drastic shift away from revascularization may now be excessive and overlook patients with “high-risk” clinical disease and resistant hypertension. Recent reports highlight the potential benefits for carefully selected patients.
