Two important concepts are highlighted by the work of Perak et al. First, not all demographic groups experience the same extent of risk accumulation over time (red arrow versus green arrow). Our goal should be to move the clinical course of those in the accelerated risk trajectory towards the attenuated risk trajectory to reduce demographic disparities (grey dashed arrow). Second, the use of novel biomarkers may offer the potential to improve patient health by measuring risk at earlier ages and lower levels of disease (A’ versus A) to enable earlier initiation of phenotype-targeted pharmacologic and non-pharmacologic interventions.
