Background: Both, acute and chronic pancreatitis represent complex disease patterns, whose effective treatment is based on structured diagnostics and therapy by a multi-professional team. There are different systems for an improved objectivity in the classification of the severity of the disease OBJECTIVES: Overview of the common classifications of acute and chronic pancreatitis.

Materials and methods: Literature research of currently used classifications of acute and chronic pancreatitis. Evaluation of the current chronic pancreatitis guideline.

Results: For acute pancreatitis, the modified Atlanta Classification and the "determinant-based" classification are most widely used. These classifications are complemented by clinical risk scores such as the Marshall Score and the SOFA Score. In chronic pancreatitis, the image-based Cambridge classification was established. For clinical assessment further classifications such as the ABC classification and the M‑ANNHEIM classification are applied evaluating leading symptoms such as pain, exocrine and endocrine pancreatic insufficiency.

Zusammenfassung: HINTERGRUND: Sowohl die akute als auch die chronische Pankreatitis stellen komplexe Krankheitsbilder dar, deren effektive Behandlung auf einer strukturierten Diagnostik und Therapie durch ein multiprofessionelles Team fußt. Für die bessere Objektivierbarkeit und klinische Umsetzung existieren hierfür unterschiedliche Klassifikationssysteme.

Fragestellung: Überblick über die gängigen Klassifikationen der akuten und chronischen Pankreatitis.

Material und methoden: Literaturrecherche der gegenwärtig verwendeten Klassifikationen der akuten und chronischen Pankreatitis sowie Auswertung der aktuellen Leitlinie „Chronische Pankreatitis“.

Ergebnisse: Für die akute Pankreatitis finden die modifizierte Atlanta-Klassifikation sowie die „determinant-based“ Klassifikation die breiteste Anwendung. Ergänzt werden diese Klassifikationen durch klinische Risiko-Scores wie den Marshall-Score und den SOFA-Score. Bei der chronischen Pankreatitis hat sich früh die rein bildgestützte Cambridge-Klassifikation etabliert. Für die klinische Einordnung mit Leitsymptomen wie Schmerzen, exokriner und endokriner Pankreasinsuffizienz dienen weitere Klassifikationen wie die ABC-Klassifikation und die M‑ANNHEIM-Klassifikation.
