Abstract

When a lung cancer patient develops an organ failure, the intensity of the care should be decided taking into account patient's wishes and his plan of carekeeping, in mind that the objective of an intensive care unit (ICU) admission is to allow the patient to be discharged from ICU and hospital with an acceptable quality of life. But the physician in charge of the patient at the time of acute disease often does not have these information. It is therefore essential that the referring oncologist had an early discussion with the patient to inform him and collect his opinion. These information have to be noted in the patient's medical chart. The prognostic criteria of lung cancer patients admitted in ICU are related to the patient's characteristics, the cancer's characteristics and the severity of acute disease. In order that a decision of ICU admission is in accordance with the patient's therapeutic project, a close discussion between the oncologist and the intensivist is essential, especially in this period of SARS-CoV2 pandemy.© 2021 SPLF. Lorsqu’un patient atteint de cancer broncho-pulmonaire (CBP) présente une défaillance d’organe, l’intensité des soins à mettre en place doit tenir compte des souhaits du patient et de son projet de soins en n’oubliant pas que l’objectif d’une admission en réanimation est de permettre au patient de sortir de réanimation et de l’hôpital avec une qualité de vie acceptable. Mais le médecin en charge du patient lors de l’épisode aigu n’a souvent pas ces informations. Il est donc indispensable que l’oncologue référent du patient ait eu une discussion anticipée avec le patient pour l’informer et recueillir son avis et que ces informations soient notées dans le dossier médical du patient. Les critères pronostiques des patients avec un CBP admis en réanimation sont liés aux caractéristiques du patient, du cancer et à la gravité de la situation aiguë. Pour que la décision d’admission ou non en réanimation soit conforme au projet thérapeutique du patient, une discussion étroite entre l’oncologue et le réanimateur est indispensable, surtout en cette période de pandémie par le SARS-CoV2.
