Rurality is defined by rural-urban commuting codes (micropolitan = primary flow within a population of 10 000-49 999 people; small rural = primary flow within a population of 2500-10 000 people; isolated rural = primary flow outside of an area with >2500 people). Concordance is defined as the location for definitive hospitalization matching the child’s primary residence in level of rurality. Odds ratios are adjusted for age, state, and level of rurality. Whiskers indicate 95% CIs.
