Sarcoid granuloma with a core composed of “foamy” neutral lipid–laden macrophages, demonstrating the enrichment of mechanistic target of rapamycin complex 1 (mTORC1), sterol regulatory-element binding protein 1a/c (SREBP1a/c) and its transcription factor (SREBF1), macrophage receptor with collagenous structure (MARCO), and the Janus kinase–signal transducer and activator of transcript (JAK-STAT) signaling pathways. Macrophage-derived monocytes obtained from patients with chronic sarcoidosis cluster spontaneously in vitro. Clustering and aberrant lipid metabolism is decreased after treatment with a lipoprotein-deficient serum and a statin (lovastatin). The Tsc2 knockout mice had thickening of skin overlying the hind paws and tail and increased pulmonary inflammation, which both improved after administration of a cholesterol-free diet and statin treatment.
