Abstract

We would like to present a 49-year-old female patient who was presented with a vulva lesion and palpable inguinal lymph nodes who were diagnosed with disseminated multiorgan involvement of high grade diffuse large B-cell lymphoma. The 18F-fluorodeoxyglucose positron emission tomography computerized tomography imaging showed multiple cervical, axillary, and abdominal lymph nodes, pulmonary nodules as well as gross hypermetabolic vulvar lesion.

A 49-year-old female patient with complaint of vulvar swelling attended for gynecologic evaluation to the hospital. The physical examination verified vulvar lesion as well as inguinal lymph nodes. The pathologic example was obtained and additional F-18 FDG PET/CT examination was performed with the suspicion of pelvic malignancy. The imaging was performed after fasting for 6 h and 11 mCi radiopharmaceutical injection as a whole body examination in the craniocaudal direction by PET/CT scanner (Siemens MCT). The imaging showed multiple enlarged hypermetabolic lymph nodes all over the body, pulmonary nodules, kidney, bone, and spleen lesions as well as vulvar hypermetabolic lesion. Whole-body horizontal PET and CT-fused image [Figure 1a] and transaxial plane image of vulva [Figure 1b]. Lymphoma involvement of the vulva is an extremely rare condition. There are several case reports presented in the literature previously.[1234] F-18 FDG PET/CT images of a patient with vulva involvement were presented as a case report;[5] however, in that case, the lesion was as a single vulvar lesion. This presented case report showed multiple organ involvement of lymphoma with unexpected presentation of vulvar lesion.
