Tunisian Recommendations for Best Practices in the Management of Spondyloarthritis: Methodology

This framework was developed under the auspices of the Tunisian League Against Rheumatism (LITAR), coordinated by a project leader. The primary objective is to formulate recommendations for the management of spondyloarthritis, grounded in the development of questions structured according to the PICO model. This model defines four essential elements of a clinical question: P: Patient or Population or Problem, I: Intervention (the proposed action), C: Comparison (between diagnostic tests, treatments, etc.), and O: Outcome (clinical results, measured events, judgment criteria, etc.). A working group was established, coordinated by a project leader appointed by LITAR. This group included rheumatologists recognized as experts in the disease by the scientific community, as well as a methodological and epidemiological rheumatologist. The bibliographic search focused on international literature (meta-analyses, randomized controlled trials, cohort studies, case-control studies, etc.) and other sources of information (HAS, clinical practice guidelines), as well as national literature (theses, dissertations, publications, conference presentations). In-person and virtual meetings of the working group enabled the formulation of the recommendations, following the presentation of literature data and extensive discussions among the experts. The draft of the recommendations was submitted to the same experts for validation and assessment of the level of agreement, which determined the strength of the recommendations. Subsequently, the recommendations were reviewed by a reading group appointed by the working group. The final version was adjusted based on feedback from both groups.

Ce référentiel a été élaboré sous l’égide de la ligue tunisienne anti rhumatismale (LITAR) coordonné par un chef de projet. L’objectif général est la production de recommandations de prise en charge de la spondyloarthrite, avec comme socle de travail une élaboration des questions et formulation selon le modèle PICO qui définit les 4 éléments d’une question clinique (3): P : Patient or Population or Problem I : Intervention (ce qu’on veut faire) C : Comparison (entre tests diagnostiques médicaments, …) O : Outcome (résultat clinique, événement mesuré, critère de jugement,..). Un groupe de travail a été constitué, coordonné par un chef de projet désigné par la Litar. Ce groupe comprenait des médecins rhumatologues considérés comme experts de la maladie par la société savante, et un rhumatologue méthodologiste et épidémiologiste. L’analyse de la littérature a porté sur la période : mars 2016-mai 2020, à l’aide des mots clés adaptés, dans les bases de données (MeSH) et une recherche manuelle dans certaines situations avec évaluation du niveau de preuve. La recherche bibliographique a porté sur la littérature internationale (méta analyses, essais comparatifs randomisés, études de cohortes, études cas-témoins,…) et auprès d’autres sources d’information (HAS, guides de pratique clinique) ainsi que sur la littérature nationale (thèses, mémoires, publications, communications affichées). Des réunions physiques et à distance du groupe de travail ont permis l’élaboration du libellé des recommandations, après exposé des données de la littérature et discussion entre les experts. Le texte des recommandations a été soumis aux mêmes experts, pour validation et notation du degré d’accord, donnant la force de la recommandation. Les recommandations ont ensuite été soumises à un groupe de lecture, désigné par le groupe de travail. La version finale a été modulée en fonction des remarques des deux groupes.
