Abstract

The frequency of occurrence of ano-perineal fistulas, especially in the context of inflammatory bowel diseases, makes them a subject of growing interest, mainly because of the discomfort and medico-social impact they induce in patients who carry them. Their treatment depends on their extent and potential for evolution. MRI appears as the technique of first choice for their assessment and follow-up. After a brief technical review, we'll look at the anatomy and imaging assessment of ano-perineal fistulas in terms of inflammatory activity and healing, two important characteristics that could influence both the prognosis and the therapeutic strategy.

La fréquence de survenue des fistules ano-périnéales, particulièrement dans le contexte de maladies intestinales inflammatoires, en fait un sujet d’intérêt croissant, principalement à cause de l’inconfort et de l’impact médico-social qu’elles induisent chez les patients qui en sont porteurs. Leur traitement dépend de leur extension et de leur potentiel évolutif. C’est dans ce contexte qu’intervient l’utilisation de l’IRM comme examen d’imagerie de premier choix pour leur évaluation et leur suivi. Après un bref rappel technique, nous approcherons l’anatomie et l’évaluation par imagerie des fistules ano-périnéales en termes d’activité inflammatoire et de cicatrisation, deux caractéristiques importantes susceptibles d’influencer le pronostic et la stratégie thérapeutique.
