The guidelines on Helicobacter pylori (HP) have changed considerably. HP should always be considered a persistent and not a transient infection. A positive HP test should always be treated in order to prevent complications such as gastric or duodenal ulcers and gastric carcinoma. The prevalence of HP has been decreasing for many years but remains high among immigrants from many countries. Because the antimicrobial resistance situation has deteriorated significantly, we always recommend resistance testing before first-line therapy.

Einleitung: Die Guidelines zu Helicobacter pylori (HP) haben stark geändert. HP soll immer als persistierende und nicht als vorübergehende Infektion gewertet werden. Bei einem positiven HP-Test soll immer therapiert werden, um Komplikationen wie Magen- bzw. Duodenalulzera und das Magenkarzinom zu verhindern. Die HP-Prävalenz nimmt seit vielen Jahren ab, bleibt aber bei Immigrant/-innen aus vielen Ländern hoch. Weil sich die Resistenzlage deutlich verschlechtert hat, empfehlen wir schon vor Erstlinientherapie immer eine Antibiotikaresistenztestung. Introduction: Les directives concernant Helicobacter pylori (HP) ont fortement changé. HP doit toujours être considéré comme une infection persistante et non comme une infection passagère. En cas de test HP positif, un traitement doit toujours être mis en place afin de prévenir les complications telles que les ulcères gastriques ou duodénaux et le carcinome gastrique. La prévalence de l’HP diminue depuis de nombreuses années, mais reste élevée chez les immigrant.e.s de nombreux pays. Comme la situation en matière de antibiorésistance s’est nettement détériorée, nous recommandons toujours un test de résistance avant même le traitement de première ligne.
