Abstract

Endobronchial intubation with a single-lumen endotracheal tube (ETT) is a technique to provide one-lung ventilation in small children. The shorter length of the right mainstem bronchus poses challenges while placing an ETT on the right side. Although the recommendation is to position the Murphy's eye of an ETT to approximate with the right upper lobe bronchus orifice, this allows only a small segment of the ETT to be placed in the bronchus, with a risk for tube dislodgement. A modification of elongating the Murphy's eye can enable the tube to be placed more distally while maintaining ventilation to the right upper lobe.
