Case Report of a Leaflet Tear Due to Dehiscence of Surgical Mitral Valve Ring Treated by Combining Edge-Edge Repair With Paravalvar Leak Occluder

Abstract

This describes a percutaneous repair in the context of a patient with failed surgical mitral valve repair, where the ring dehiscence caused a hole at the base of the posterior mitral valve leaflet. Redo surgery was considered too high risk, so we combined edge-edge repair with device occlusion of the hole in the posterior leaflet, using a Nitinol wire device designed for paravalvar leak closure. Whilst mitral valve repair is usually a successful procedure, rare complications can occur, and percutaneous solutions can offer an alternative to high-risk surgery, where medical treatment has failed. The patient was discharged and remains well under follow-up.
